You will use trauma-informed care, Housing First, and motivational interviewing to support participants toward permanent housing, with monthly home visits and ongoing documentation per program contracts.

Qualifications

• Bachelor’s Degree in Social Work or related field is required; experience with housing and case management techniques is preferred.
• Two years of case management experience and HMIS familiarity are preferred.
• Bilingual (Spanish) is a plus.

Responsibilities

• Provide field-based, client-centered case management focused on housing stabilization and budgeting.
• Guide clients through housing searches, applications, leases, and move-in.
• Develop individualized service plans addressing short- and long-term goals.
• Apply Trauma-Informed Care, Housing First, Motivational Interviewing, and Harm Reduction practices.
• Collaborate with clients to update housing stability plans and budgets.
• Maintain documentation and compliance with program policies (HIPAA).
