Job summary

Advantage Home Care is seeking a registered nurse to lead quality improvement initiatives across hospice and home health services in Albuquerque, NM. You will oversee policy implementation, mentor staff, and collaborate with the Director of Clinical Operations to support patient care and regulatory compliance.

You will conduct and participate in regular quality improvement meetings, supervise documentation accuracy, and ensure service coordination while maintaining compassionate, family-centered

Qualifications

• A registered professional nurse with a current NM license.
• Knowledge of Medicare/Medicaid Hospice and Home Health regulations.
• At least one year of nursing experience in a community health service setting within the past six years.
• At least one year experience in Quality Assurance.
• Demonstrated ability to manage terminal and home health care with crisis intervention and family counseling.
• Strong written and oral communication, supervision of staff, documentation requirements, and service coordination.

Responsibilities

• Lead quality improvement meetings and monitor initiatives.
• Review, revise, and improve quality initiatives and ongoing measurements.
• Mentor and role model for staff; collaborate with Director of Clinical Operations.
• Assist in staff meetings for information and staff support as indicated.
• Ensure compliance with agency policies and licensing regulations.
• Maintain accuracy and completeness of medical records and documentation.
