Qualifications

• A registered professional nurse with a current license to practice in the State of New Mexico.
• Demonstrates knowledge regarding Medicare/Medicaid Hospice and Home Health regulations.
• At least one year of nursing experience in a community health service setting within the past six years.
• At least one year experience in Quality Assurance.
• Demonstrated ability to manage all aspects of terminal care and home health care with expertise in crisis intervention, family counseling, pain and symptom control and bedside nursing.
• Demonstrated skill in administrative performance including communication, supervision of staff, documentation requirements, and service coordination.

Responsabilités

• Provides for regular Quality Improvement meetings for review, revision, and improvement and ongoing measurement of Hospice and Home Health quality initiatives.
• Implements and monitors compliance with agency policies and procedures.
• Serves as a mentor and role model.
• Collaborates with Director of Clinical Operations to provide staff meetings for information and staff support as indicated.
• Makes visits to patient/families as indicated by patient/family need and staff need for supervision and/or direct nursing care.
• Participates in the supervision of the maintenance of medical records, assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.
• Promotes the utilization of resources for the benefit of team members, patients and families consistent with agency procedures.
• Identifies need for staff education and assistance in the development of inservice programs for interdisciplinary team.
• Demonstrates knowledge of nursing practice.
• Develops and implements policies and procedures that guide and support the provision of services.
• Continuously assesses and improve the agencies performance.
• Maintain quality improvement programs, as appropriate.
• Supports agency's philosophy, goals, objectives, mission, vision and values.
• Participates in quality improvement to improve processes and patient care.
• Schedules support staff to participate in quality improvement initiatives.
• Implements actions, as appropriate, recommended by quality improvement initiatives.
• Accept other assignments as may be appropriate.
