Job summary

Advantage Home Care in Albuquerque, NM seeks a dedicated Registered Nurse to evaluate and treat patients using current technology and practices. You will communicate with physicians to decide the initial plan of care based on assessments and orders.

You will monitor clinical outcomes, participate in CQI initiatives, educate Home Health Aides and LPNs, and spend more time with patients to make a meaningful difference in their lives.

Qualifications

• Current and unrestricted RN license in the state of practice.
• Minimum of two years hospice experience preferred.
• Current CPR certification.
• Nurse Licensure Compact (NLC) documents the mutual recognition for multi-state practice.
• Ability to assess patient status and identify age-specific needs.
• Reliable transportation and valid driver’s license.

Responsibilities

• Evaluate and treat patients using current technology and practices.
• Communicate with the physician to decide the initial plan of care based on assessment and orders.
• Participate in clinical outcomes monitoring, follow-up and CQI initiatives.
• Spend more time with patients and make a meaningful difference in their lives.
• Manage and educate Home Health Aides and LPNs on nursing services.
• Gain quality experience working with a multi-disciplinary team and personal patient interaction.
• Provide timely response to after hours/weekend/holiday requests for assistance.
• Demonstrate understanding of CQI process and participate in CQI.
