Job summary

Mercer University’s Mercer Medicine is seeking a Quality Care Coordinator to support clinics by coordinating patient transitions from hospitals and other settings back to primary care. The role focuses on timely post-discharge follow-up, care coordination, medical record review, quality-measure tracking, and identifying care gaps to promote continuity of care.

• Candidates must complete an application.

Responsibilities

• Transitional Care Management (TCM) - Check every day to see which patients were recently in the hospital, ER, or another healthcare facility.
• Figure out which patients need follow-up help after being discharged.
• Call patients soon after they get home to check on them.
• Ask whether they have their medications, understand what they need to do, and have their follow-up appointments scheduled.
• Schedule their primary-care appointment within the required timeframe.
• Make sure the doctor and care team know what happened during the hospital stay and what the patient needs.
• The overall goal is to keep patients from getting sick again and ending up back in the hospital or ER unnecessarily.
• Quality Management / Performance Improvement - Look at reports to see which patients are missing recommended healthcare services.
• Help the doctors' office identify what each patient is missing and get those things completed.
• Keep spreadsheets, lists, and reports updated.
• Help the organization meet its healthcare quality goals and requirements.
• Help gather and double-check information that is used for quality reports and potentially affects reimbursement/payment.

Job Description

The Quality Care Coordinator supports our clinics by coordinating patients’ transitions from hospitals, emergency departments, and other healthcare settings back to primary care. This position is responsible for timely post-discharge follow-up, care coordination, medical record review, quality-measure tracking, and identification of patient care gaps. The Coordinator works closely with patients, providers, specialists, hospitals, and health plans to promote continuity of care, improve outcomes, reduce avoidable readmissions, and support value-based care initiatives.

Requirements

• Candidates will be invited to complete an application based on block performance, PBL evaluation comments and faculty recommendations.
• Pay Rate: $15.00 per hour
• Scheduled Hours: 20
• Start Date: 09/14/2026
• End Date: 05/31/2027
