The Association for Individual Development (AID) is seeking a Case Manager Qualified Mental Health Professional Intake for Behavioral Health to provide assessment, intake, counseling, and case management services to clients with persistent mental illness. This role supports adults in our community and works within our DHS/CARF compliant framework.

• Master's in a human services field is required.
• 1+ year of behavioral health experience preferred.
• Experience with DHS/CARF standards is a plus.

Responsabilités

• Provision of assessment, counseling, skills training and case management services to a caseload of clients with persistent mental illness.
• Complete Intake/IM+CANS and other required paperwork.
• Conduct new client intakes including IM+CANS and LOCUS Assessments.
• Maintain prescreening/intake logs.
• Provide documentation of services per DHS and CARF.
• Advocate for and maintain rights of clients served.
• Assign clients to case managers’ caseloads in coordination with program managers.
