Job summary

Association for Individual Development in Elgin, IL is seeking a Case Manager Mental Health Professional or QMHP to develop and implement IM+CANS-based treatment plans and coordinate services. The role requires collaboration with a team of behavioral health providers and adherence to agency and state standards.

Qualifications

• Bachelor’s degree in Human Services or related field required for MHP; Master’s degree for QMHP.
• Experience working in social services is required.
• Ability to work collaboratively with teams and external providers.

Responsibilities

• Develop and implement individual treatment plans using IM+CANS to maximize client independence.
• Complete documentation of mental health services per state and agency standards.
• Coordinate with internal staff and external agencies to ensure client-centered care.
• Attend required supervision and team meetings and maintain professional boundaries.
• Provide transportation or client support as needed and ensure program goals are met.
