Qualifications

• Must be at least 18 years of age.
• A bachelor's degree in human services is required. Recommended bachelor's degrees include psychology, sociology, special education, rehabilitation counseling or therapy, social work, certified occupational therapist, and registered nurse ETC.
• Possess a valid driver's license and automobile insurance.
• At least one year of experience working with individuals with disabilities preferred

Responsibilities

• The QIDP/Program Coordinator is responsible for the whole person using clinical case management.
• They provide person-centered support and oversight in the development, implementation, and monitoring of individualized service plans for individuals with intellectual and developmental disabilities, ensuring compliance with federal and state regulations.
• Leverage expertise in behavioral health, program coordination, and interdisciplinary collaboration to promote independence, dignity, and quality of life for individuals served.
