Job summary
EP!C is seeking a compassionate Qualified Intellectual Disabilities Professional (QIDP) to provide person-centered case management and program oversight for individuals with intellectual and developmental disabilities.
You will develop individualized service plans, coordinate interdisciplinary supports, monitor Host Home services, ensure regulatory compliance, and advocate for the individuals we serve. The ideal candidate will be committed to dignity and quality of life.
Qualifications
- Bachelor's degree in human services or related field per Illinois DHS QIDP standards.
- Strong organizational, communication, and time management skills for case management.
- Ability to document accurately and maintain compliance with policies.

- Provide person-centered case management by developing, implementing, and monitoring individualized service plans.
- Oversee Host Home providers and residential services with site visits and assessments.
- Coordinate medical, behavioral health, psychiatric and community supports.
- Ensure compliance with Illinois DHS regulations, HCBS Settings Rule, HIPAA, and mandated reporting.
- Collaborate with individuals served, families, guardians, teams, and community partners.

- Oversee Host Home providers and residential services by completing site visits, environmental audits, assessments, and ensuring individuals receive high-quality support that meet agency and DHS standards.
- Coordinate medical, behavioral health, psychiatric, Community Day Services, and other community-based supports to ensure each individual's needs and preferences are met.
- Maintain compliance with Illinois DHS regulations, HCBS Settings Rule, HIPAA, agency policies, and mandated reporting requirements while ensuring all documentation, electronic records, and assessments are accurate and completed on time.
- Collaborate with individuals served, families, guardians, interdisciplinary teams, Host Home providers, and community partners to promote positive outcomes and continuity of care.

- Valid Driver's License and reliable transportation with automobile insurance
- Strong organizational, communication, and time management skills
- Ability to manage multiple priorities and maintain accurate documentation
- Professional interpersonal and ethical decision-making skills
- Proficiency with Microsoft Office and electronic documentation systems
- Ability to work independently while collaborating with interdisciplinary teams
- 1 year of experience supporting individuals with intellectual and developmental disabilities (IDD)
- Experience as a QIDP or Case Manager
- Knowledge of Illinois DHS/CILA regulations and HCBS Settings Rule
- Experience developing person-centered plans and behavioral supports
- Experience coordinating interdisciplinary services and residential programming
- Equivalent education and experience will be considered where applicable under DHS requirements.
- Frequent walking and sitting
- Regular reaching, grasping, and standing
- Other physical activities related to an office environment
- Operates general office & lift equipment
- Ability to lift 50 lbs
