Job summary

Central Plains Center in Plainview, TX is seeking a full-time Case Manager - QIDP to coordinate services for individuals with Intellectual and Developmental Disabilities. This role requires strong assessment and planning skills, with the ability to monitor care plans and maintain accurate records.

Education options include a Bachelor's in social or related field, an Associate degree, or a high school diploma with relevant experience; QIDP eligibility is required.

Qualifications

• Must have one of the following degrees: Bachelor's in social/behavioral/human services, or an Associate in related field, or a HS diploma with 2 years experience with individuals with IDD.
• Must be able to qualify as a Qualified Intellectual Disabilities Professional (QIDP).
• Preferred: at least 1 year experience with individuals with IDD; valid TX driver's license and insurability.

Responsibilities

• Performs service coordination to individuals, managing crisis situations when needed.
• Performs assessments, monitors needs, plans services and coordinates referrals.
• Monitors and evaluates care plans and maintains up-to-date client records.
