Job summary

EP!C is seeking a QIDP/Program Coordinator to provide person-centered case management for individuals with IDD, developing and monitoring service plans that promote independence and dignity. You will supervise Host Home providers and conduct site visits to ensure high-quality supports meeting DHS standards.

Coordinate medical and behavioral health services, maintain compliance with regulations, and collaborate with families and teams to achieve positive outcomes for those served.

Qualifications

  • Bachelor’s degree preferred in Human Services or education with supervisory experience.
  • Minimum one year experience with persons with developmental disabilities.
  • Ability to read and comprehend simple instructions and write correspondence; strong communication skills.

  • Oversee Host Home providers and residential services through site visits, audits, and DHS-aligned quality.
  • Coordinate medical, behavioral health, and community supports to meet individuals’ needs and preferences.

  • Maintain compliance with Illinois DHS regulations, HCBS Settings Rule, HIPAA, agency policies, and mandated reporting requirements while ensuring all documentation, electronic records, and assessments are accurate and completed on time.

  • Collaborate with individuals served, families, guardians, interdisciplinary teams, Host Home providers, and community partners to promote positive outcomes and continuity of care.

Requirements

Education/Experience Requirements


  • Bachelor’s Degree preferred in Human Services or education and two years’ supervisory experience required.

  • Supervisory experience is essential.

  • One year experience with persons with developmental disabilities.

  • The ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write correspondence.

  • Ability to effectively present information in one-on one and small group situations to customers, clients, and other employees of the organization.
