Job summary

The Association for Individual Development (AID) in Elgin, IL is seeking an experienced Case Manager QIDP to coordinate care for individuals with I/DD across Health, Behavioral Health, Employment, and Transportation services. You will manage a caseload of up to 20 clients, document services, lead planning meetings, and ensure compliance with DHS, HCBS, and CARF standards.

Flexible hours within a Monday–Friday schedule, with some evenings/weekends as needed to support client needs; the role

Qualifications

• Bachelor's degree in a relevant field.
• Experience with I/DD service programs.
• Ability to coordinate multiple services across departments.

Responsibilities

• Manage caseload of up to 20 clients.
• Maintain client records in electronic system.
• Schedule and lead service planning meetings.
• Coordinate Health/Behavioral/Employment/Transportation services.
• Ensure DHS/HCBS/CARF compliance and documentation.
