Job summary

EP!C seeks a compassionate and organized Qualified Intellectual Disabilities Professional (QIDP) to provide person-centered case management and program oversight for individuals with intellectual and developmental disabilities.

This role develops individualized service plans, coordinates interdisciplinary supports, monitors Host Home services, ensures regulatory compliance, and advocates for the individuals we serve, upholding quality, dignity, and person-centered care.

Qualifications

  • Bachelor's degree in human services or a related field qualifying under Illinois DHS QIDP standards.
  • Valid Driver's License and reliable transportation with automobile insurance.
  • Strong organizational, communication, and time management skills.
  • Ability to manage multiple priorities and maintain accurate documentation.
  • Professional interpersonal and ethical decision-making skills.
  • Proficiency with Microsoft Office and electronic documentation systems.
  • Ability to work independently while collaborating with interdisciplinary teams.

Responsibilities

  • Provide person-centered case management by developing, implementing, and monitoring individualized service plans that promote independence, dignity, and quality of life for individuals with IDD.
  • Oversee Host Home providers and residential services by completing site visits, environmental audits, assessments, and ensuring individuals receive high-quality support that meet agency and DHS standards.
  • Coordinate medical, behavioral health, psychiatric, Community Day Services, and other community-based supports to meet each individual's needs and preferences.
  • Maintain compliance with Illinois DHS regulations, HCBS Settings Rule, HIPAA, agency policies, and mandated reporting requirements while ensuring all documentation, electronic records, and assessments are accurate and completed on time.
  • Collaborate with individuals served, families, guardians, interdisciplinary teams, Host Home providers, and community partners to promote positive outcomes and continuity of care.
