Job summary

EP!C is seeking a compassionate Qualified Intellectual Disabilities Professional (QIDP) to provide person-centered case management and program oversight for individuals with intellectual and developmental disabilities.

You will develop individualized service plans, coordinate interdisciplinary supports, monitor Host Home services, ensure regulatory compliance, and advocate for the individuals we serve. The ideal candidate will be committed to dignity and quality of life.

Qualifications

• Bachelor's degree in human services or related field per Illinois DHS QIDP standards.
• Strong organizational, communication, and time management skills for case management.
• Ability to document accurately and maintain compliance with policies.

Responsibilities

• Provide person-centered case management by developing, implementing, and monitoring individualized service plans.
• Oversee Host Home providers and residential services with site visits and assessments.
• Coordinate medical, behavioral health, psychiatric and community supports.
• Ensure compliance with Illinois DHS regulations, HCBS Settings Rule, HIPAA, and mandated reporting.
• Collaborate with individuals served, families, guardians, teams, and community partners.
