Job summary

PossAbility, LLC seeks a Qualified Intellectual Disability Professional (QIDP) to oversee ISPs for individuals with diverse disabilities in Illinois. You will develop and implement behavior support plans, prepare monthly reviews, and collaborate with an interdisciplinary team to ensure compliant, person-centered services.

The role emphasizes data-driven progress, case management, training staff, and ensuring IDHS regulations are followed.

Qualifications

Bachelor's degree in healthcare, human services, public health, or related field.
QIDP certified and listed on the Department’s QIDP database.
Five+ years of experience as a direct care worker for individuals with IDD.
Ability to support individuals with IDD and develop support strategies.
Knowledge of home care regulations and best practices.
Strong leadership and communication skills.
Valid driver’s license and proof of insurance.
Current First Aid and CPR certifications.

Responsibilities

ISP Development and Implementation: develop, implement, and monitor ISPs to meet individuals' needs.
BSPs: assist in developing and overseeing behavior support plans.
Monthly Program Reviews: prepare written reviews, data collection, and narrative updates.
Interdisciplinary Collaboration: work with therapists, nurses, and social workers.
Regulatory Compliance: ensure programs comply with IDHS and other regulations.
Case Management: serve as primary case manager for individuals.
Training and Supervision: train staff and supervise others.
