Job summary

ViaQuest is seeking a QIDP to join ViaQuest Healthcare Central, supporting individuals with developmental disabilities in our ICF facilities. The role is full-time and on-call for incident reporting, with a salary between $50,000 and $53,000 annually.

You will maintain knowledge of state and federal regulations, ensure Individual Plan implementation, monitor medications and appointments, and support residents through assessments and planning meetings.

Qualifications

• Four-year degree in social services or related field required.
• One year of experience may substitute for experience.
• Strong customer service and communication skills.
• Supervisory or management experience preferred.

Responsibilities

• Maintain knowledge of state and federal rules for ICF/IID operations.
• Ensure proper Individual Plan implementation at day programming.
• Monitor medications and medical supplies management by staff.
• Monitor appointments with outside providers as appropriate.
• Complete and maintain quarterly Individual Assessments with documentation.
• Conduct annual and special Individual Plan meetings for residents.
• Update floor book information to reflect current status.
• Prepare all Plans, assessments, reviews and meeting minutes.
• Coordinate use of community services based on resident needs and preferences.
