Merakey in Chico, CA is seeking a Qualified Intellectual Disabilities Professional (QIDP) to join our Intermediate Care Facilities. The role involves developing progress notes, assessments, and evaluations required by regulations, and mentoring the support team.

You will collaborate with clinicians, Direct Support Professionals, and regional center partners to implement ISPs and train staff, coordinating care for individuals with intellectual and developmental disabilities.

Responsibilities

• Collaborate with the Merakey team to create and implement all Individual Service Plans
• Conduct training related to the ISPs for the consumer and Direct Support Professionals
• Work within multidisciplinary settings with physicians, nurses, direct support staff, etc.
• Provide ongoing communication with the Regional Center’s liaison or case worker regarding the consumer’s status
