Job summary

Hartford HealthCare at Home is seeking a QAPI Specialist to advance clinical documentation, outcomes, and safety through a systematic data-driven approach. You will compile trends, report quality data, and lead performance improvement plans across patient care, infection control, risk management, and customer satisfaction.

You will review records, monitor compliance with federal/state standards, and educate staff to reduce variation in practice while promoting culturally competent care.

Qualifications

• 3 years of certified home health experience.
• Value Based Purchasing, PDGM, CAHPS-preferred familiarity.
• CT Licensure in field of specialty.
• Strong data analysis and reporting skills.
• Ability to design and implement performance measures.
• Willingness to travel to branches nationwide as needed.

Responsibilities

• Develop and implement performance improvement plans using evidence-based guidelines.
• Collect, analyze, and report quality data across patient care, safety, risk, infection control, outcomes and satisfaction.
• Lead audit activities and serve as regulatory resource for clinical staff.
• Monitor OASIS accuracy and appropriate ICD-10 coding related to patients.
• Provide education and feedback to staff on quality and safety practices.
• Support timely, compliant data submissions to payors and regulators.
