Job summary

Evolve Therapy Services seeks a licensed Physical Therapy Assistant for an in-house Skilled Nursing Facility in Minneapolis, MN. The PRN position supports the Physical Therapist in delivering resident-focused PT services in a licensed setting.

The role involves screenings, implementing treatment plans, and documenting CMS-compliant care in NetHealth. Active licensure and ongoing professional education are required to maintain standards and ensure quality resident outcomes.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide PT services to residents under direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in establishing target goals and treatment plans with the Physical Therapist.
• Conduct individual and group treatments as outlined in the plan and document appropriately.
• Coordinate schedules and educate residents, families and staff on therapy programs.
