Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant to provide in-house skilled nursing facility therapy under the supervision of a Physical Therapist in Burlington, Wisconsin. This PRN position supports planning, evaluation, and delivery of therapy across resident populations in a facility setting.

The role emphasizes screenings, treatment planning, and execution of therapy modalities, while ensuring CMS/NetsHealth documentation and compliance.

Qualifications

• Associates degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in identifying goals and treatment plans with the Physical Therapist.
• Conduct individual and group treatments as outlined in treatment plans (strength, mobility, gait, ROM, modalities).
• Assist in selecting PT techniques/media and adapt as indicated by resident performance.
• Monitor residents’ response to intervention and consult with PT for modifications.
• Assist in establishing/modifying resident schedules with the Physical Therapist and team.
• Adhere to department/resident schedules and adjust for regimen.
• Educate residents, families, and staff in use of equipment and therapy strategies.
• Document compliant and medically necessary CMS/NetsHealth documentation.

Education
