Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Burlington, WI. This PRN position works under the supervision of a Physical Therapist to deliver patient-centered therapy and support resident mobility and function.

Responsibilities include conducting screenings, implementing treatment plans, documenting progress in NetHealth EMR, and collaborating with the care team to ensure quality resident care.

Qualifications

• Associate's degree in physical therapy.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluating residents and developing treatment plans with the Physical Therapist.
• Implement treatment plans and conduct individual and group therapies as outlined.
• Monitor residents’ responses to therapies and report modifications to the Physical Therapist.
• Assist in establishing and modifying resident schedules with the team.

Education

Associate's degree in physical therapy from an accredited program

Active Physical Therapy Assistant license in state(s) of practice
