Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Florida. The role supports a licensed Physical Therapist in delivering resident-focused therapy services and developing treatment plans.

The PTA will conduct screenings, participate in therapy sessions (individual and group), and help coordinate schedules, documentation, and staff education to ensure high-quality resident care.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in identifying targets, goals, and treatment plans with the Physical Therapist.
• Conduct individual and group treatment as outlined in the plan, including mobility, gait, ROM, and modalities.

Education
