Job summary

Evolve Therapy Services, LLC is seeking a Physical Therapy Assistant to support in-house Skilled Nursing Facility services in the Tampa area. The role provides direct resident care under the Physical Therapist, including screenings, treatment implementation, and performance monitoring.

Ideal candidates hold an active PTA license, an Associate's degree in physical therapy, and commit to ongoing professional development to maintain licensure. Full-time, part-time, or PRN shifts may be available.

Qualifications

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Perform screening to determine need for intervention or treatment.
• Assist in evaluating resident progress and treatment planning.
• Train residents and staff in equipment usage and safety.
• Maintain CMS compliant and medically necessary documentation in NetHealth.

Responsibilities

• Develop target treatment plans with short- and long-term goals.
• Conduct individual and group therapy sessions as assigned.
• Monitor residents’ response to therapy and adjust plans as needed.
• Coordinate schedules with the Physical Therapist and care team.
