Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Waverly, NE. This PRN role supports the Physical Therapist in delivering high-quality resident care within a multidisciplinary team.

Responsibilities include screenings, assisting with treatment plans, and documenting outcomes in NetHealth EMR. The position emphasizes ongoing professional development and adherence to CMS guidelines.

Qualifications

• Must hold an active Physical Therapy Assistant license in state(s) of practice.
• Associate's degree in physical therapy from an accredited program.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention or treatment.
• Assist in establishing targets, goals and treatment plans with the Physical Therapist.
• Deliver individual and group treatment as outlined in the plan, focusing on strength, mobility, balance, gait, and ROM.
• Monitor resident responses and modify interventions with PT input.
• Assist in scheduling and coordinating resident therapy sessions with the care team.
• Assist with documentation and compliance in NetHealth EMR per CMS requirements.
