Evolve Therapy Services, LLC seeks a Part Time or PRN Physical Therapy Assistant for an in-house Skilled Nursing Facility in Blue Hill, Nebraska. The role supports the Physical Therapist with direct patient care, screenings, and treatment planning within CMS guidelines.

Responsibilities include scheduling, education for residents and families, and accurate documentation in NetHealth EMR. APTA-aligned CE and active licensure are required to maintain high standards of care.

Formación

• Associate’s degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Responsibility for ongoing continuing education and professional development.

Responsabilidades

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention or treatment.
• Assist the Physical Therapist in evaluations and treatment planning.
• Assist in establishing resident schedules with the team.
• Train residents, families and staff in use of equipment and treatments.
• Documentation in NetHealth EMR per CMS requirements.
