Job summary

Evolve Therapy Services in Sutherland, NE is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility on a PRN basis. You will work under the supervision of a Physical Therapist to provide therapy services, conduct screenings, and support clinical goals for residents in a structured care environment.

The role requires an Associate's degree in physical therapy, an active PTA license in the state(s) of practice, and ongoing continuing education.

Qualifications

• Must hold an active Physical Therapy Assistant license in state(s) of practice.
• Associate's degree in physical therapy from an accredited program.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluation and in developing treatment plans with the Physical Therapist.
• Plan and implement individual and group treatment sessions.
• Train residents, families and staff in use of therapy equipment.
• Maintain compliant and medically necessary documentation in NetHealth EMR.
