Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Central City, NE. The role operates under the supervision of a Physical Therapist to deliver comprehensive therapy services.

The PTA will conduct screenings, help develop treatment plans, and provide both individual and group sessions tailored to residents’ needs. Knowledge of NetHealth EMR is beneficial; licensure must be active in practice states.

Qualifications

• Must have an Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluating residents under the direction of the Physical Therapist.
• Assist in establishing individual resident schedules with the Physical Therapist and team.
• Conduct individual and group treatment including mobility, strength, gait, balance, ROM and modalities.
• Assist in CMS-compliant documentation in NetHealth EMR systems.
