Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Grand Island, NE. This PRN position supports the Physical Therapist with resident care and treatment planning.

Responsibilities include conducting screenings, following treatment plans, and documenting progress in NetHealth EMR. Requires an associate degree, active PTA license, and ongoing professional development.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist in establishing and modifying individual resident schedules in conjunction with the Physical Therapist and other team members.
• Assist in selecting PT techniques/media and adapt as indicated by resident performance.
• Monitor residents’ response to intervention and modify as needed.
