Job summary

Flagstar Rehab is seeking a Physical Therapy Assistant (PTA) to join our team in Skilled Nursing Facilities. The role offers flexible schedules, per diem options, and independent contracting guidance.

You will work under supervision, provide services to SNF patients, and participate in weekly pay and benefits as applicable. Requirements include NYS PTA license, BLS, and EMR proficiency, with eligibility to work in the US.

Qualifications

• Current NYS PTA license and registration.
• Valid BLS certification.
• EMR competence and fluency with electronic medical records.
• Meets all State and Federal requirements for US employment in a Nursing Facility.

Responsibilities

• Collaborate daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to SNF patients with conditions like TBI, CVA, Stroke, Parkinson's, MS, cancer, and orthopedic surgery.
• Monitor and document patient progress.

Hourly Compensation:

• W2: $34-$36
• 1099: $38-$40

Physical Therapy Assistant Requirements and Responsibilities:

• Current NYS PTA License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
