Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist Assistant to join our patient-centered team. This PRN role is based at La Crescent Health Services in La Crescent, MN, working under the supervision of a licensed PT to deliver quality care.

You will implement individualized treatment plans, document patient progress, educate patients on home exercise programs, and assist with devices. Prior PTA experience is preferred; licensure must be current in MN.

Qualifications

• Associate degree PTA required.
• Current, unencumbered MN PTA license.
• 1 year PTA experience preferred.
• Implements individualized treatment plans under PT supervision.
• Documents patient progress and reports changes to supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.

Physical Therapist Assistant Requirements & Responsibilities

• Associate Degree in Physical Therapist Assistant Program required


• Current, unencumbered state Physical Therapist Assistant License


• 1 year of previous experience as a Physical Therapist Assistant preferred


• Implements individualized treatment plans under the supervision of a Physical Therapist


• Documents patient progress and reports changes in patient status to the supervising PT
