Job summary

Continuum Therapy Partners, LLC in Taylorville, IL seeks a dedicated Physical Therapist Assistant to join our care team on a PRN basis. You will work under a supervising PT to deliver individualized treatments and support patient progress.

Key responsibilities include implementing treatment plans, educating patients on home exercise programs, and documenting status changes to ensure continuity of care at Taylorville Care Center.

Qualifications

• Associate Degree in Physical Therapist Assistant Program required.
• Current, unencumbered state Physical Therapist Assistant License
• 1 year of previous experience as a Physical Therapist Assistant preferred.

Responsibilities

• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.
