Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist Assistant to join our team at Monroe Manor Nursing & Rehab Center in Monroe, Wisconsin. This PRN daytime position supports our therapist-led approach and helps deliver high-quality patient care.

We are looking for candidates with an Associate Degree in PTA, an active PTA license in Wisconsin, and a preference for those with at least one year of PTA experience.

Qualifications

• Associate Degree in Physical Therapist Assistant Program is required.
• Current, unencumbered state PTA license is required.
• 1 year of PTA experience is preferred.

Responsibilities

• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.
