Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Glendale, WI. This PRN position supports the Physical Therapist by delivering patient-centered care, performing screenings, and assisting with evaluations and treatment planning.

Qualifications

• Requires Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice is mandatory.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide PT services under supervision of a Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluations and in developing treatment plans with the PT.
• Assist in establishing resident schedules with the PT and team.
• Train residents and staff in equipment use and therapy techniques.
• Document in NetHealth EMR per CMS expectations.
• Maintain high quality resident care and productivity during shift.

Education
