Evolve Therapy Services in Fort Atkinson, WI, is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility on a PRN basis.

You will provide therapy services under the supervision of a Physical Therapist, perform screenings, contribute to treatment planning, and support resident goals and education.

Required: Associate’s degree in physical therapy, active PTA license, and ongoing continuing education to maintain licensure. Benefits vary by status.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluating residents under the direction of the Physical Therapist.
• Help develop target goals and treatment plans.
• Conduct individual and group treatment as outlined in the plan, including strength, mobility, balance, gait, ROM, and modalities.
• Assist in selecting therapy techniques and sequences, adapting as needed.
• Monitor residents’ responses and modify as directed by the Physical Therapist.
• Maintain resident schedules and documentation in CMS-compliant EMR (NetHealth).
• Educate residents, family, and staff as needed.
