Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Fort Atkinson, WI. This PRN role supports the Physical Therapist to deliver resident-focused care and therapy services on-site.

You will perform screenings, assist in evaluations, and implement treatment plans, including strength, mobility, gait, balance, and modalities while maintaining CMS-compliant documentation. Active PTA license required and ongoing education encouraged.

Qualifications

• Must have an Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying target goals and treatment plans.
• Conduct individual and group treatments as outlined in the treatment plan covering strength, mobility, gait, balance, ROM, and modalities.
• Assist in selecting PT techniques/media and sequence of activities; adapt as indicated.
• Monitor residents’ response and consult PT if modification is needed.
• Establish and modify resident schedules with the PT and team members.
• Train residents, family, and staff in equipment use; participate in education activities.
• Maintain compliant and medically necessary documentation per CMS expectations in NetHealth EMR.
