Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Fort Atkinson, WI. This PRN position involves assisting the Physical Therapist, delivering therapy services, and contributing to treatment plans.

You will screen residents, implement treatment plans, train residents and staff, and ensure CMS-compliant documentation in NetHealth EMR. Requires an Associate's degree in PT and PTA license.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the direction of the Physical Therapist.
• Assist in identifying targets and goals in treatment plans.
• Conduct individual and group treatment as outlined in the treatment plan, including strength, mobility, gait, ROM, and modalities.
• Assist in selecting PT techniques/media and adapt as indicated.
• Monitor residents’ response to intervention and modify as needed.
• Assist in establishing and modifying resident schedules with the PT and team.
• Adhere to department and resident schedule and adjust as needed.
• Train resident, family and staff in equipment use.
• Participate in resident, family, and staff education.
• Maintain CMS-compliant documentation in NetHealth EMR.

Education

Associate's degree in physical therapy

PTA license in state(s) of practice
