Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Silvis, IL. The PTA will provide therapy services under the direct supervision of a Physical Therapist, conducting screenings and contributing to treatment plans.

This PRN role emphasizes flexible scheduling and collaboration with the healthcare team. Eligible candidates hold an Associate's degree in Physical Therapy and an active PTA license, with ongoing continuing education

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluations under the direction of the Physical Therapist.
• Assist in establishing and modifying resident treatment plans and goals with the Physical Therapist and team.
• Complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems.
