Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Silvis, IL. This PRN position supports the physical therapy team under the supervision of a PT, conducting resident screenings and implementing treatment plans to improve mobility and function.

You will provide individual and group therapy, help educate residents and families, and ensure documentation meets CMS expectations using NetHealth EMR. Active PTA license is required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation under the direction of the Physical Therapist.
• Assist in establishing/ modifying treatment plans with the Physical Therapist.
• Conduct individual and group treatment as outlined in the plan.
• Document CMS-required information in NetHealth EMR.
