Job summary

Evolve Therapy Services in Freeport, IL, is hiring a Physical Therapy Assistant to support an in-house Skilled Nursing Facility. You will provide PT services under the supervision of a Physical Therapist, contribute to treatment plans, and help residents improve mobility, strength, balance, and function.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the Physical Therapist.
• Assist in establishing treatment plans with short- and long-term goals.
• Conduct individual and group treatment addressing strength, mobility, balance, gait, ROM, and modalities.
