LionStone Care is seeking a dedicated Physical Therapist Assistant to support physical therapy services in Akron, OH. This role collaborates with the Physical Therapist, supports Telehealth, and ensures timely evaluations, progress notes, and discharges.

The position requires an Associate degree as a Physical Therapist Assistant and involvement in care planning, documentation, and resident safety. Eligible for student loan repayment assistance powered by Clasp.

Qualifications

• Must possess an Associate degree from an accredited program conferring the title of Physical Therapist Assistant.

Responsibilities

• Assist the Physical Therapist in planning, development, organization, evaluation, and directing therapy activities.
• Facilitate Telehealth services to ensure evaluations, recertifications, progress notes, and discharges are completed on time.
• Communicate with staff from related departments when planning therapy services.
• Assist in maintaining physical therapy standards of practice and documentation.
• Ensure notes are informative and recorded on the date of service.
• Provide individualized PT services according to the Plan of Care.
