Job summary

Carle Health is seeking a Licensed Physical Therapist Assistant to implement plans of care under a PT, serve as a clinical resource, and educate patients and staff across ages. The role includes a sign-on bonus and potential relocation assistance for eligible candidates.

The ideal candidate holds an Illinois-licensed PTA credential with BLS and board eligibility, and will follow the PT plan of care while documenting communications and patient progress in the therapy EMR.

Qualifications

• Licensed Physical Therapist Assistant (IDFPR) in Illinois.
• Basic Life Support (BLS) within 30 days - American Heart Association.

Responsibilities

• Educates patients, family, caregivers, students, and other health care providers.
• Follows the therapist's plan of care and documents communications.
• Completes daily documentation and therapy EMR charges.
• Coordinates with PT on patient treatment interventions.
• Communicates progress and recommendations to referring providers.
