Job summary

Elara Caring in Portage, IN is seeking a highly motivated Physical Therapist Assistant to join our home health team. You will deliver prescribed therapy in patients’ homes and monitor progress toward mobility and independence goals.

Responsibilities include educating patients and caregivers on home programs, assisting with device use, and ensuring safe, high-quality care under the supervision of a Physical Therapist. Valid license and reliable transportation are required.

Qualifications

• Current, unrestricted PTA license valid for the state of work.
• Graduate of an accredited PTA program approved by the American Therapy Association.
• 6+ months experience as a Physical Therapist Assistant is preferred.
• Ability to lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver's license or have reliable transportation to travel between patient homes.

Responsibilities

• Provide physical therapy services according to the treatment plan established by the Physical Therapist.
• Help patients work toward therapy goals that improve mobility, function, and independence.
• Monitor patient progress and report changes to the supervising Physical Therapist.
• Educate patients and caregivers on home therapy programs and techniques to support continued progress.
• Teach patients and families how to safely use assistive devices, including canes, walkers, wheelchairs, prosthetics, and orthotics.
• Deliver safe, high-quality therapy services in accordance with physician orders, clinical standards, and state practice guidelines
