Job summary

Elara Caring is seeking a licensed Physical Therapist Assistant to deliver home health PT services. You will work under a supervising Physical Therapist to help patients improve mobility and independence in their homes on Cleveland’s east side, Euclid and surrounding communities.

The role requires travel about 50% and a valid Ohio PTA license, with preference for Homecare Homebase experience. Join a team dedicated to high-quality, patient-centered care.

Qualifications

• Graduation from an accredited Physical Therapist Assistant program.
• Current, unrestricted PTA license valid in Ohio.
• 6 months or more of PTA experience preferred.
• Home health/homecare software experience (Homecare Homebase) preferred.
• Ability and willingness to travel approximately 50% of the time.
• Valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

• Provide physical therapy services according to the Plan of Care established by the supervising Physical Therapist.
• Review the care plan before each visit and incorporate patient goals into treatment.
• Monitor patient response and report changes to the Physical Therapist.
• Teach patients and caregivers how to follow their home therapy program.
• Instruct patients in the safe use of walkers, canes, and other assistive devices.
• Document care accurately and follow Ohio regulations.
• Collaborate with the care team to support quality and continuity of care.
