Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist Assistant for an in-house Skilled Nursing Facility in Reading, MA. The PTA will work under the supervision of a Physical Therapist to deliver resident-centered therapy services and participate in care planning.

Responsibilities include screenings, treatment planning, documenting in NetHealth EMR, and educating residents, families, and staff. Requires an active PTA license and ongoing continuing education.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist in establishing and modifying individual resident schedules with the Physical Therapist and team.
• Train residents/family/staff in equipment use and participate in education.
• Complete CMS-mandated documentation in NetHealth EMR systems.
