Job summary

Evolve Therapy Services is hiring a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Webster, MA. The role supports the Physical Therapist in delivering patient-centered care and adheres to CMS documentation standards.

Responsibilities include screenings, plan development, and both group and individual treatments focusing on mobility, strength, balance, and gait, with guided supervision. Active licensure and ongoing education are required.

Qualifications

• Associate's degree required from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist in identifying targets and treatment plans with short- and long-term goals.
• Conduct individual and group treatment in areas like strength, mobility, balance, gait, ROM, and modalities.
• Assist in selecting techniques/media and adapt as indicated with guidance from the Physical Therapist.
• Monitor residents’ response and consult on modifications with the Physical Therapist.
• Assist in scheduling with the Physical Therapist and other team members.
• Train residents and families in equipment use; participate in education of residents and staff.
• Maintain compliant and medically necessary documentation in NetHealth EMR as required by CMS.
