Job summary

Home Health Care, Inc. is seeking a Physical Therapist Assistant to provide one-on-one therapy in patients' homes across St. Cloud and Sauk Centre areas. You will implement individualized plans of care under a supervising PT, focusing on functional mobility and safe home environments.

You'll travel to patients, deliver therapeutic exercises, monitor progress, and document outcomes while collaborating with an interdisciplinary team. Minnesota PTA license required.

Qualifications

• Active Minnesota PTA license is required.
• Strong therapeutic exercise and functional mobility skills.
• Excellent patient and caregiver communication.
• Ability to travel throughout St. Cloud & Sauk Centre territory.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide one-on-one Physical Therapy treatments in patients' homes.
• Carry out individualized plans of care.
• Provide therapeutic exercise and strengthening.
• Perform gait and mobility training.
• Improve balance, coordination, transfers, and endurance.
• Educate patients and caregivers on safe mobility and therapeutic techniques.
• Monitor patient response and tolerance to treatment.
• Communicate changes in patient condition to supervising PT.
• Update supervising PT on patient progress.
• Collaborate with nurses, OTs, SLPs, physicians, and other clinicians.
• Complete timely clinical documentation.
