Qualifications

• Must have Associates degree from an accredited Physical Therapy Assistant program.
• Must possess and maintain CPR certification.
• Must have valid driver’s license and ability to travel to all locations.
• Experience with home health care and interdisciplinary teams preferred.

Responsibilities

• Provides patient care and documents treatment per physician orders and agency policy.
• Document interventions, priorities and progress.
• Educates patients and families and communicates findings to therapist and physician.
• Maintain timely daily notes and ensure continuity of care.
• Meet productivity standards and participate in team meetings.
