Job summary

AccentCare in Saint Paul, MN is seeking a dedicated Physical Therapist Assistant to join our Home Health team. You will work under a licensed Physical Therapist, providing patient-focused care across home health settings and managing a caseload with proper documentation.

Responsibilities include implementing and adjusting treatment plans, educating patients and families, collaborating with the interdisciplinary team, and ensuring timely notes.

Qualifications

• Associates degree from an accredited school of Physical Therapy.
• Valid license, registration, or permit to practice in the state of operation.
• CPR certification maintained during employment.
• Must be able to travel to all business locations.

Responsibilities

• Provides patient care and documents treatment per physician orders and policy.
• Manages a caseload under supervising therapist and adheres to productivity standards.
• Educates patients and families, and communicates findings to the therapist and physician.
• Maintains timely notes and completes required forms to prevent care interruptions.
• Attends agency meetings and participates in in-services as assigned.
