Job summary

Home Health Care, Inc. is seeking a Physical Therapist Assistant to bring skilled therapy directly to patients in their homes across St. Cloud and surrounding Central Minnesota communities.

You’ll travel to patients, work one-on-one, and implement plans of care under a supervising PT. This role emphasizes independent scheduling and meaningful, in-home rehabilitation for mobility, balance, and daily function.

Qualifications

• Active Minnesota PTA license and eligibility to practice in home health.
• Strong therapeutic exercise and functional mobility skills.
• Excellent patient and caregiver communication is essential.

Responsibilities

• Provide one-on-one Physical Therapy treatments in patients' homes under supervision.
• Carry out individualized plans of care and document progress.
• Perform gait and mobility training, balance and endurance work.
• Educate patients and caregivers on safe mobility and techniques.
• Coordinate with the supervising PT and interdisciplinary team.
