Job summary

Continuum Therapy Partners, LLC is seeking a qualified Physical Therapist for a PRN role in Wisconsin Rapids. You’ll join a team that values your clinical voice and supports high-quality patient care with flexible scheduling.

Requirements: Doctor of Physical Therapy, current WI license, NPTE, and preference for 1 year of PT experience. Shifts are daytime PRN at Wisconsin Rapids Health Services and River Run Assisted Living in Wisconsin Rapids.

Qualifications

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the NPTE
• 1 year of previous experience as a Physical Therapist preferred

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
